Becoming a part of the Sunshine
Care family requires just a few simple steps.
Please feel free to contact
us if you would like more information or have any questions. Our admission
process is described below:
1.
Prior to moving in, each resident is required
to have a completed Physician
Report. This report must include a diagnosis,
medication orders,
documentation of a TB test; list of allergies;
and a description of functional
and cognitive capabilities. We will provide
you with the form, or it can be emailed
to the physician.
2.
Following a review of the Physician's Report
and a physical assessment by Sunshine
Care, the Admission Agreement will be signed
and a move-in date will be
established.
3.
A deposit is collected to secure the room
for 30 days.
4.
Upon admission, the deposit is credited
to the first month’s rent.
